When a Batch Fails: Testing-Failure Recovery

A failed COA is a fork in the road, not a dead end — how to read a microbial fail, decide between resample, remediation, and destruction, and run the root-cause trace back to the dry room that actually caused it.

A fail is data, not a verdict

The testing lesson established the gate: potency, microbial, and pesticide/solvent panels, an Aspergillus action limit around <10,000 CFU/g, TYM against jurisdictional limits (USP ≤10⁴ CFU/g), and a 95%+ pass rate as the real KPI. This lesson is what you do the day a lot doesn't pass. The wrong move — panic-destroying a salvageable lot, or worse, quietly re-testing until you get a pass without fixing anything — costs money and, in the second case, a license. Run the decision like an investigation.
